Case managers work with the remainder of the clinical and medical team and with each client in finding the best fit. This is usually a gender-specific treatment program, and sometimes one that focuses on dual diagnosis treatment, is age-specific or focuses on trauma-informed care.
  • Thorough and personalized aftercare planning – Aftercare planning focuses on more than rehab placement. Our case managers help prepare for the remainder of the recovery process, carefully laying out a fool-proof strategy and helping clients along every step of the way. Preventing relapse down the road is one of our top priorities.
